What do I need to bring to my appointment?

1. IDENTIFICATION: Mandatory
Please bring a photo ID such as an Arkansas driver’s license or state-issued ID card. You MUST be 18 years of age and older and be an Arkansas resident to receive a medical marijuana card in Arkansas. Our clinic will only see patients over the age of 21 for evaluation.

2. DOCUMENTATION: Must Plan Ahead
Please bring supporting documentation with your diagnosis or prior treatment to the clinic at the time of your appointment. Documentation should clearly state your name, visit date, qualifying diagnosis, and treatment.

Other useful items you may bring can be ER / Hospital Discharge Papers, Medication Bottles with your name and RX on the label. It is important to provide all available supporting documentation to validate your condition.

3. PAYMENT: Is Due at Time of Service
Your signed marijuana certification, if approved, will be provided to you immediately after your visit and after full payment is made.

What method of payments do you accept?

  • We accept cash and major credit cards
  • We do NOT accept personal checks
  • Insurance is not accepted

What do I do with the signed qualification letter?

After evaluation and approval from our qualifying physician, you will receive a physician-signed certification document. With this document, you can apply for your marijuana card at the Arkansas Department of Health (ADH) website or send your completed Arkansas marijuana card application directly to the state. The ADH charges a $50 fee to be included with your application.

Can your doctor diagnosis me with a qualifying condition?

No. There are pre-qualifying diagnosis that are established for the state of Arkansas. The doctor at Arkansas Medical Cannabis Network can only certify you for one or more previously established diagnosis. There must be prior treatment with traditional medicine before a qualifying condition can be treated with medical marijuana. Existing conditions must have a track record including diagnosis. This documentation can be obtained from your other doctors including primary care, specialists, and ER doctors.
